Job Description

About the role
As the first point of contact for the service, you will play a vital role in supporting residents to access the help they need to live safely and independently in their homes. You will gather information, assess needs, and determine the most appropriate next steps, ensuring that cases are triaged effectively and allocated to the right professionals.

Responsibilities

The role requires strong organisational skills, excellent attention to detail, and the ability to manage a varied workload. You will need to demonstrate professional curiosity, taking the time to fully understand people’s circumstances, identify risks or unmet needs, and ensure that the best possible outcomes are achieved, particularly for vulnerable residents.

You will work closely with colleagues across the service, as well as a range of partners, to provide a coordinated and responsive approach that supports independent living.

  • A customer-focused approach, with experience of supporting or working with vulnerable people
  • The ability to assess information, make informed decisions, and communicate clearly
  • A high level of professional curiosity to ensure risks are identified and needs are fully understood
